Secondary Stroke Prevention

In 2015, as part of the Provincial Integrated Work Plan Process, the Regional Stroke Networks identified the need to update the secondary prevention clinic (SPC) model of care as outlined by the Joint Stroke Strategy Working Group in their report to the Ministry of Health and Long-Term Care (MOHLTC, 2000). Specifically the regions identified the need to create a model which better reflected current best practices and provincial needs.

To guide this work, a group of clinical experts from across the 11 Regional Stroke Networks was formed. This working group was tasked with producing 3 key deliverables:

1. A list of Core Elements recommended for SPCs in Ontario

2. A list of Evaluation Indicators to support SPCs to measure their performance against these Core Elements, and

3. An implementation toolkit to support organizations to prioritize and implement the core elements
