Mohan, age 69, was upset to learn that recent bloodwork revealed an elevated eGFR, meaning that his kidneys were not functioning properly. The screening program offered at his primary care clinic enabled him to learn about treatment options and self-management approaches to reduce the risk of progression of his chronic kidney disease.

How might our primary care team improve the processes within our clinic to support chronic kidney disease screening for more patients?
