eGFR = Estimated Glomerular Filtration Rate

ACR = Albumin to Creatinine Ratio

Primary Care settings with limited resources/capacity should focus on high risk

	% of adult patients 18 years of age or older with identified risk factors such as diabetes*	# of eligible patients who refuse screening
% of adult patients eGFR >30 or urine ACR < 60 with identified risk factor(s)* in the EMR	% of adult patients with identified risk factors such as diabetes* with eGFR or Urine ACR value in the EMR
