The Reason for the Effort

  • Carol, age 67, was unaware that her blood pressure was uncontrolled and this has resulted in the decline of kidney function. Imagine being able to screen, engage, and follow the ‘Carols’ in your practice to ensure this does not happen?
  • Ahmed, age 38, has a family history of high blood pressure but was shocked to learn that he has high blood pressure at his age. The screening program offered at his primary care clinic enabled him to learn about treatment options and self-management approaches to reduce risks for his vascular health.

How might our primary care team improve the processes within our clinic to identify patients with hypertension sooner and support hypertension management for more patients?
