Patient Toolkit (working document)

The Patient Toolkit is a curated list of well-developed provincial, national, and international tools and resources on lower-limb, foot and wound prevention and management for providers to share with patients and their families to guide and support their lower-limb preservation education, understanding and self-management. This Patient Toolkit was developed with patients and family advisors who provided feedback, input, and guidance to ensure meaningful resources were included.

Care providers, programs and organizations are encouraged to develop a plan (e.g., a user-friendly platform) for sharing clear and consistent lower-limb preservation educational tools and resources with patients and families to optimize self-management, improve outcomes and preserve limbs.

The Patient Toolkit will be expanded and co-developed by CorHealth and lower-limb preservation stakeholders to support provincial scale-up.
