Section 3: Follow-Up and Prevention
A stroke survivor is vulnerable to many transition points as they progress through the acute, sub-acute and chronic stages of recovery, interacting with a range of care providers in several different health-care settings. Communication between these care providers and care settings is critical for ensuring patient safety and quality of care. Evidence shows that when there is coordination of care beyond the inpatient setting and community support services are provided, patient outcomes and patient and caregiver satisfaction improves.
