  % of adult patients 18 years of age or older with a BP value in the EMR*  
% of adult patients diagnosed with hypertension and with BP value ≤140/90  or ≤ 135/85 via AOBP**  % of adult patients 18 years of age or older with a diagnosis of hypertension  

*BP values may be obtained during regular scheduled office visits or may be provided by patients in follow-up to BP measurement at another location, e.g. community pharmacy.

**AOBP=Automated Office Blood Pressure

MEASURES: High-Risk Factors Focus*

Screening, Identification & Management
Primary Care settings with limited resources/capacity should focus on high risk

  % of adult patients 18 years of age or older with identified risk factors such as diabetes*  
% of adult patients with identified risk factor(s)* with BP value ≤ 140/90  (≤ 130/80 for patients with diabetes) in the EMR  % of adult patients with identified risk factors such as diabetes* with BP value in the EMR  

*Risk factor examples include:

  • Ethnicity-Southeast Asian, African, Aboriginal
  • Diabetes
  • Vascular diseases-CVD, CKD, PAD, CAD
  • Behavioural activities- e.g., smoking
  • Other-Obesity
