*BP values may be obtained during regular scheduled office visits or may be provided by patients in follow-up to BP measurement at another location, e.g. community pharmacy.

*Risk factor examples include:
- Ethnicity-Southeast Asian, African, Aboriginal
- Diabetes
- Vascular diseases-CVD, CKD, PAD, CAD
