Managing Stroke Care Transitions


In 2010, key recommendations for ‘Managing Stroke Care Transitions’ were established to facilitate and support increasingly seamless, integrated care and improved outcomes for patients and their families and/or caregivers across the continuum of care. The Transitions recommendations of the Canadian Stroke Best Practice Recommendations has been updated in 2016.

Areas of emphasis include:

  • Support through transitions
  • Patient and family education
  • Interprofessional communication
  • Discharge planning
  • Early supported discharge
  • Community reintegration following stroke

A number of implementation resources and knowledge transfer tools have been identified
