Demonstration Program Engagement Approach

A demonstration program is a regional, interprofessional group of lower-limb preservation providers, organizations, champions and patient and family advisors from across the care continuum who are formally committed to implementing changes to improve outcomes for individuals at risk of non-traumatic major lower-limb amputation.

CorHealth is engaging with demonstration programs to test implementation of components of the Ontario Framework for Lower-Limb Preservation. Demonstration program engagement will also include measurement and reporting and funding policy components to support successful evaluation, broader implementation, and sustainability. Demonstration programs will be identified based on expressed interest and readiness for lower-limb preservation change and will represent diverse geographic locations and populations. Lessons learned from demonstration program engagement will further shape the Strategy to support provincial scale-up.
